I have sent a Nonrefundable deposit of $100.00 to Keren Herrera on ________________ 20__, in a form of Personal check/Cashier check (please circle one). The remaining balance of $700.00 dlls is due at time of pick up of puppy at 7 1/2 // 8 weeks old.

I understand that this litter comes with Limited Registration.
(NO BREEDING RIGHTS)

On day of pick up I will be receiving with my puppy, his/her puppy packet containing everything I need to know about my new addittion to my family.  Plus a copy of this form with the date on it of when it was received, a receipt/ and a Contract for my puppy.

If you are Interested in a specific puppy and you don't think that this Page with your Nonrefundable deposit will get here in time, Please fill out the boxes below with the specific information required & submit. This will allow me to get it fast & place your puppy on Temp. Hold. While your Non-refundable deposit with this page filled gets to me. Once received the puppy will be moved from Temp. Hold to Reserved. But Please do not forget to Print this page fill out & send it with your Nonrefundable deposit, it has to arrive to me in a week, since it causes other families to be turned away from getting that puppy.
If I do not receive it, the puppy will be removed from
